PATIENT INFORMATION Room Service
The Nutritional Services staff will prepare your meals to order. A room service menu is at your bedside. Call extension 6033 to place your order between 7 AM and 5:30 PM. If you need assistance, please ask your nurse. Your meal will be prepared and delivered within approximately 30 minutes. Snacks are available at any time from your nurse.
Calling Your Nurse
Your nurse will explain how to use the nurse call button in your room or bathroom. Each button turns on a light above the door to your room and rings on handheld phones carried by nursing staff for quick response to your needs. We will respond to your call as soon as possible.
Room Temperature
Each patient room contains its own heating/cooling unit located by the window. You may adjust the temperature to meet your comfort needs.
Telephones
There is a telephone in each patient room. Local calls are free, and long distance calls can be made with a credit card or third party calling.

Cell phones are allowed in your room. If you would like to rest and not be disturbed, please tell your nurse, and he or she will hold your calls. Family and friends may call a patient’s room by dialing (509) 682-3300 and asking for the nurses’ station or your room number.
Television
A television with basic cable is provided for each patient. Handheld remotes are available. Please ask your nurse for assistance as needed. Cable stations are listed on in your patient guide.
PATIENT SERVICES At Lake Chelan Community Hospital, we strive to provide services to make your stay as comfortable as possible.
Fun Cart
A fun cart with games, books, toys, movies, music and audio books is available to patients and their guests. Please ask your nurse for a list of available items. If you are interested in any item on the list, please ask your nurse or contact Volunteer Services at ext. 6346. The items will be delivered directly to your room. When you are done with the items, simply let your nurse know, and she will return them to Volunteer Services.
Mail
Mail will be delivered to a patient’s room. If we receive mail after you are discharged, it will be forwarded to your home address.
Flowers and Balloons
Flowers, balloons and other gifts will be delivered to a patient’s room. Due to patients and staff who may have sensitivities to latex, we will not accept latex balloons. Mylar (foil) balloons are acceptable.
Language Assistance
If you or a family member needs a language interpreter, please tell us immediately upon registration. We have interpretive services available 24 hours a day.
Environmental Services
Each floor has dedicated Environmental Service Personnel assigned to it. Your housekeeper will clean your room once a day. If you are in need of additional housekeeping services or desire extra blankets, towels or pillows, please contact your nurse. Lake Chelan Community Hospital is committed to using green cleaning supplies throughout the hospital.
Birthdays and Anniversaries
Should your birthday or anniversary occur during your stay with us, please notify your nurse. We would like to honor you by making sure your special day is celebrated.
OPEN MEDICAL RECORDS
As a leader in patient-centered care, the staff of Lake Chelan Community Hospital are happy to share the contents of your medical records with you. A member of your healthcare team will gladly walk you through your medical records and answer any questions you may have.
VISITOR INFORMATION Visiting Hours & Guidelines
We recognize the value of family and friends in the recovery process. Therefore, guests are allowed in patient rooms at all hours. We do ask, however, that visitors check in at the nurses’ station before going into a patient’s room. We also ask that people with colds, sore throats or any other contagious disease not visit patients. Guests may be asked to wait outside the patient room during tests and other procedures. If a patient becomes tired, he or she may tell the nurse to limit visitors. You may enter from either the lower or upper entrance between 6 AM and 6 PM M-F. At other times, please enter through the upper emergency room and registration entrance.
Guest Meals
Guests are welcome to dine in the hospital’s Orchard Café. It is located on the first floor, across from the elevator. Our daily menu is posted on the hospital’s website at www.lakechelanhospital.com. The menu is also available on bulletin boards throughout the hospital. Hot food is available during the following hours:

Snacks and beverages are also available in the vending machine on the second floor.
Bragg Educational Center
The Bragg Educational Center was dedicated in grateful appreciation and recognition of Mr. and Mrs. Harold Bragg, who bequeathed the majority of their estate, over $1 Million, to the Lake Chelan Community Hospital Foundation. The funds are to be used exclusively to provide scholarships to residents of the Lake Chelan Valley who are either pursuing careers in the healthcare fields - or are local healthcare professionals involved in continuing medical education. The Bragg Educational Center is open to patients, family, community members and healthcare providers to research medical information and provide teleconference support for meetings and educational sessions. A computer is available to access medical information online. Guests may visit the center, unless the room is in use for a continuing medical education class or other meeting.

Nursing
Our nursing staff is deeply committed to giving you personal, warm and attentive care; it is composed of registered nurses (RN), licensed practical nurses (LPN) and certified nurse assistants (CNA). All nurses are highly skilled and trained to care for you. Because we are a rural hospital, we have the opportunity to provide very personal and individualized patient-focused care. It is our goal to meet and exceed the expectations and needs of our patients and their families.
Surgical Center
As part of our full range of healthcare services, we offer you the opportunity to have surgical procedures performed close to home. Our surgical team of board certified general surgeons, specialists and surgery staff offer general and specialized surgery for both inpatients and outpatients. We have the following surgical specialties:

Our specialty surgeons and physicians are supported by a Certified Registered Nurse Anesthetist. All surgery patients receive pre- and post-operative education to make the preparation for surgery and recovery easier. In addition, we have agreements with local resorts to provide respite for visiting patients while recovering from plastic surgery.
Emergency Room
Lake Chelan Community Hospital has a full-service emergency room (ER) certified for level IV trauma care. We offer 24-hour care with all ancillary services, including x-ray, CT scan and lab diagnostics; our emergency team includes a trauma surgeon, physicians, emergency medicine-specialized physician assistants, ER Technicians and registered nurses. A physician assistant and nurse are always on duty, while a physician is on call. The ER team is trained in Advanced Cardiac Life Support, Pediatric Advanced Life Support, Neonatal Resuscitation and Trauma Nurse Core Course, as well as Advanced Trauma Life Support. Because of the high quality of emergency medical care at LCCH, the ER has become a training center in rural emergency medical care.
Emergency Medical Services
Since 1987, Lake Chelan Community Hospital Emergency Medical Services has provided prompt, quality emergency medical care and transportation. We are a community-supported public service that provides paramedic-level advanced life support to residents and visitors.
Physicians
At Lake Chelan Community Hospital, we are privileged to have excellent physicians. We have family medicine doctors, as well as the specialties of women’s health, general surgery, foot and ankle surgery, vascular surgery, plastic surgery and optometry.
Diagnostic Imaging/Radiology
We have the technological ability to provide high quality diagnostic exams here, close to home, and send those images and reports to your doctor. We are part of a larger medical network and regularly send images to facilities in North Central Washington, as well as to hospitals and doctors in Spokane and Seattle. Diagnostic Imaging has become an increasingly important tool for your doctor to detect and more accurately treat your medical condition. More than just a traditional x-ray, our diagnostic imaging can assist your doctor while you're at Lake Chelan Community Hospital. Highly trained, fully licensed radiology technologists perform each diagnostic test. An in-house 16-slice CT scanner enables doctors to handle an emergency or medical condition with precision and accuracy; mobile x-ray equipment allows images to be taken during surgery. Our visiting MRI service is the most advanced imaging technology in north central Washington. These, along with general, advanced x-ray, ultrasound and mammography services, are part of our expert care.
Your physician will order imaging that is necessary for your treatment. Some examinations may require that you fast before your test. Others will require a full bladder or a special drink. Your nurse and/or physician will explain any preparation required for your procedure. Please call extension 6285 or ask your nurse for assistance if you have any questions.
Laboratory Services
At Lake Chelan Community Hospital, the laboratory is an essential part of your total health care. The laboratory is staffed by highly educated and trained medical laboratory technicians and technologists who perform routine testing. Laboratory services are available to patients at the hospital seven days a week. After-hour testing and emergency testing is available 24 hours a day by on-call personnel. Lab personnel will work closely with other hospital providers to ensure you well coordinated care. We are also proud to be one of the first rural hospitals in north central Washington with Tele-laboratory capabilities. This technology allows us to quickly collaborate with outside specialists and often contributes to earlier diagnoses. Respiratory Care
The department is staffed with a Registered Respiratory Therapist experienced in oxygen therapy, aerosol medication delivery, airway clearance, exercise treadmill testing, pulmonary function spirometry, high-risk obstetrics, neonatal resuscitation, trauma, respiratory assessment and patient education.
Nutritional Services
We offer personalized, outpatient nutrition therapy for individuals needing help with:
• General good nutrition for all age groups • Weight management • Diabetes • Disease states (e.g. hypertension, cardiovascular, gastrointestinal) • Food intolerances and allergies
Our goal is to provide our clients with nutrition information and tools to maximize health and prevent disease. Call 682-3300 for an appointment.
Childbirth Services
From childbirth education to a positive birthing experience, we are dedicated to providing families with a warm, relaxed atmosphere to experience one of life’s greatest moments. We combine the personal attention only a rural hospital can offer with the state-of-the-art medical care you and your baby need. Our private, remodeled birthing rooms have a homelike appearance, while being fully equipped medical units. A trained OB nurse is staffed 24 hours a day to provide care in the following: prenatal testing, labor and delivery and OB emergencies. If a Cesarean birth is necessary, our surgical team is prepared to respond. After delivery, babies and mothers stay together in one room, where they can get to know one another in a relaxed, quiet atmosphere. We can help coordinate a water birth if you desire; please contact us at least two months before your due date to make plans. A new father receives one complimentary meal after his baby’s birth, and we have two electronic breast pumps available for new mothers during their stay. Most families stay in the hospital for at least 24 hours. A mother/baby nurse is close by to help and answer any questions you may have during your stay. All OB nurses are trained in Fetal Monitoring, Management of Labor and Birth, ACLS, newborn care and Neonatal Resuscitation.
Wellness Services
Our goal is to not only give you excellent medical care when you are sick or injured, but to provide preventative programs as well. We offer employee health consultations to local businesses and regularly scheduled classes on a wide range of health topics, including diabetes education, smoking cessation and the medical benefits of a healthy lifestyle. The Wellness Program works closely with local health clinics to provide the community with additional health resources, as well as collaborates with local schools to assist with health fairs and provide educational opportunities in healthcare. A registered nurse is available to speak with your civic or business group on request.
The Sanctuary
Located at Lake Chelan Community Hospital, the Sanctuary is a premiere residential facility specializing in treatment of chemical dependency and co-occurring disorders for both men and women. The Sanctuary offers detox services and a safe and secure haven for patients to heal from addiction and deal with personal issues.
PATIENT SAFETY & SECURITY
During your stay at Lake Chelan Community Hospital, your safety and the safety of those around you are a primary concern to us. This is a weapon-free facility.
Identification Bracelets
Upon admission, you will be given an identification bracelet. All of your tests and procedures will be identified and cross-referenced by the name and number on your bracelet. Please leave the bracelet on during your entire stay.
Fire Drills
In compliance with local fire safety laws and hospital accreditation standards, there may be a fire or disaster drill while you are in the hospital. Please ask your nurse if you have any questions or concerns.
Bathrooms
All patient bathrooms have a nurse call button. Please alert your nurse if you feel dizzy or need any help. We also have bedside commodes if you are unable to safely and comfortably walk to the bathroom.
Preventing Falls
It may be necessary for your own safety to walk with assistance during your hospital stay. Please do not get out of bed alone until your nurse has helped you up at least once and given you the okay to move around your room. Use your nurse call button when assistance is needed.
Valuables
Personal valuables and cash should be left at home. If you arrive with them, we ask that you give them to a family member for safekeeping. The hospital is not responsible for money or items of value kept in your room. If you lose an item, please inform your nurse. Found items are kept in the Environmental Services Department.
BILLING & ACCOUNT INFORMATION
As a courtesy to you, our billing department will invoice your insurance company based on the information you provided at registration. We will continue to follow up with your insurance carrier until payment is received. During this process, you will receive a number of patient-friendly billing statements from Lake Chelan Community Hospital providing information on the status of your account. You are responsible for any deductibles, co-payments and / or co-insurance; you may pay online at www.lakechelanhospital.com.
Arrangements for payment of your hospital bill should be made before your admission. If you anticipate difficulty, please contact our financial advisor at 682- 6101 (#6361 within the hospital) to make the necessary payment arrangements. Unless prior arrangements have been made, your hospital bill is payable in full upon receipt.

Charity Care
If you have you do not qualify for any of the low or no cost insurance programs available, Lake Chelan Community Hospital will review your provided financial information (household income) to determine if you meet our internal Charity Care Program criteria. This program is based on Federal Poverty Guidelines and allows from 5% to 100% discounting on services billed. If, for some reason, there is still difficulty in meeting this financial obligation, LCCH will work with you to set up a payment plan that fits into your budget.
DISCOUNTS If you pay your balance in full within 90 days, you will receive a 15% discount.
Also, if you are a local homeowner, you may qualify for discount on your bill. The Commissioners of the hospital district take their responsibilities seriously and continue to pursue opportunities to finances the services provided in a fair manner. As such, if you pay taxes that support Lake Chelan Community Hospital, you are eligible for an annual discount for the actual amount of taxes you paid on your primary residence to support the hospital during that calendar year. Discounts, not to exceed $500 per household, will apply to patient account balances after all third party payments are received. A copy of your Property Tax Statement is required to apply the discount. Contact billing at 682-8517 for more information.
In essence, if you use the services of the hospital during the year (for anything, including laboratory tests, mammograms, medical procedures, etc…), the amount you pay in taxes can be applied to the account after your insurance company has paid its portion.
PARTNERS IN YOUR CARE Preventing Spread of Infection
Lake Chelan Community Hospital is part of the “Partners in Your Care” program. We strive to provide the best care to all patients. Now we need YOU to join our team. Our goal is proper hand hygiene 100 percent of the time. Become a partner with your doctor, nurse and all healthcare workers who enter your room by asking them, “Did you wash or sanitize your hands?” Ask the question any time one of your healthcare providers is about to touch you or touch things that are used in your care. Hand hygiene by patients, guests and staff is important to prevent the spread of infection to you, the patient. It is not rude to ask! Our healthcare workers are interested in your care and expect you to ask them about hand hygiene. We want you to ask!
Smoking/Tobacco Policy
Lake Chelan Community Hospital is a completely tobacco-free campus. This is for the safety of our patients and employees. We ask that you join in our efforts to build a healthier community by quitting this habit – or encouraging a friend or family member to quit. We understand quitting is hard but well worth it in health benefits. If you have tried before and failed, continue to try until you succeed. Ask your nurse for information about stopping tobacco use.
Pain Management
It is important for pain to be controlled in order for you to heal. If you are in pain, it is important to tell your physician or nurse. Also, if the pain is still there after you get your treatment or medication, tell you nurse so that other medications may be considered. Your nurse will often ask you five questions about your pain. Your answers will help your healthcare team create the best pain treatment plan for you.
1. Where is the pain? Make sure you tell your nurse all places you feel pain. 2. What does your pain feel like? Is it dull or sharp, throbbing, like pins and needles, stabbing or another feeling? Any information you can give your nurse will help. 3. Is the pain constant? Or does it come and go? 4. How much pain do you have? At Lake Chelan Community Hospital, we use a pain scale of 0-10, with 0 being no pain and 10 being the worst pain you can imagine. Where do you rate your pain? 5. Is there anything you can do to relieve the pain? Does a different position help? Has medication relieved the pain in the past?
LEAVING THE HOSPITAL
Your doctor will write instructions for your discharge and any medication, dietary instructions or special service you will need when you leave the hospital. Our social worker will help you make your transition back to the community a smooth one, helping you arrange for services such as home care or addressing any concerns you may have.
Discharge Instructions
You will receive written discharge instructions that explain how you should care for yourself at home. These instructions will include any medications you should take, as well as when you can return to normal activity. There will also be a number to call if you have any questions.
Social Services
Our social worker can assist in evaluating any of your discharge needs. She can provide information and arrange for various services such as home healthcare, medical equipment and home-delivered meals. She can also assist you with transfers to nursing homes and rehabilitation centers. Patient Survey
Upon discharge, you will receive a patient satisfaction survey. We ask that you take time to fill it out and return it, so that we can continue to improve and offer our patients the best care possible.
Home Care Services
Care at home is available through Lake Chelan Community Hospital’s Home Health and Hospice department. A team of skilled professionals provides nursing, therapy services and personal care in your home, supervised by your physician. With the help of our nursing staff, you can also receive intravenous medications or enteral and parenteral nutrition at home, permitting you to avoid extended hospital or nursing home stays. An interdisciplinary team of skilled professionals provides hospice care for terminally ill patients and their families. If you would like more information, please call (509) 682-8235.
ADVANCE DIRECTIVES
We respect your right to make your own medical treatment decisions. If you are 18 years of age or older and mentally competent, you have the right to make decisions about your medical treatment. You have the right to accept or refuse treatments recommended by your physician. An Advance Directive gives you the power to control decisions about your healthcare, should you become unable to communicate.
Advance Directives
An Advance Directive is a set of directions you give about the healthcare you want, should you lose the ability to make decisions for yourself. There are two types of Advance Directives:

Healthcare Directive (Living Will)
A Living Will is a document that directs your healthcare provider regarding the type of treatments you do or do not wish, should your condition become terminal.
Healthcare Power of Attorney
In the event that you become unable to make medical decisions related to your care, you may name a person to make these decisions for you. This person is called healthcare agent. The Healthcare Power of Attorney allows you to name your healthcare agent. Your agent can say what medical treatments you would want and not want, should you become unable to communicate. If you have a Healthcare Directive or Healthcare Power of Attorney, please have a copy brought to Lake Chelan Community Hospital for your records.
Washington State Death With Dignity Act The 2008 passage of Initiative 1000, the Washington State Death with Dignity Act, has raised many questions for physicians, pharmacists, counselors and hospitals statewide.
After careful, thoughtful conversations with our medical staff, board of commissioners and patients, Lake Chelan Community Hospital has chosen not to participate under the Washington State Death with Dignity Act. This means that our physicians, employees, independent contractors and volunteers shall not assist a patient in ending the patient’s life. In addition, no provider may participate on the premises of the hospital or in property owned by the hospital. Lake Chelan Community Hospital will continue to provide compassionate, high quality care to all of our patients. Any patient wishing to receive life-ending medication while a patient at this hospital will be assisted in transfer to another facility of the patient’s choice. The transfer will assure continuity of care.
PATIENT RIGHTS
Lake Chelan Community Hospital is committed to serving the needs of all patients with caring, competence and accountability. This facility holds the belief that the needs of our patients can best be served when the patient is involved in his/her care by understanding and exercising his/her patient rights. The patient has the right to be fully informed, as evidenced by the patient’s written acknowledgement, prior to or at the time of admission and during stay of these rights and all rules and regulations governing patient conduct and responsibilities. Lake Chelan Community Hospital affirms the following patient rights:
Access to Care
The right to expect that, within its capacity, the Hospital will make reasonable response to the request of a patient for services. The Hospital will provide evaluation, service, and/or referral as indicated by the urgency of the case. When medically permissible, a patient may be transferred to another facility only after he/she has received complete information and explanation concerning the need for such transfer. The Hospital may require transfer or discharge for medical reasons or for the patient’s welfare or that of other patients.
Safety The patient has a right to receive care in a safe setting.
Right to Know
The right to know that Hospital rules and regulations apply to his/her conduct as a patient. The patient also has the right to have a family member or representative of his/her choice and his/her own physician notified promptly of his/her admission to the hospital. The patient has a right to be fully informed, prior to or at time of admission and during their stay, of services available in the facility, and of related charges including any charges for services not covered by the facilities basic per diem rate. The patient has the right to be informed by a physician of his/her medical condition at all times unless medically contraindicated. The patient has the right to participate in his/her plan of care. The patient, or surrogate decision maker, has the right to be informed of unanticipated outcomes of his or her care.
Right to Transfer
The patient has a right to be transferred or discharged only for medical reasons, for his/her welfare or for that of other patients, and is given reasonable advance notice to ensure orderly transfer or discharge; such actions are documented in his/her medical record. A patient may not be transferred to another facility unless he/she has received complete explanation of the need for transfer and the alternatives to such a transfer, and unless the transfer is acceptable to the receiving facility.
Right to Voice Complaints & Recommendations
If you have a special concern or problem during your stay, we want to look into it before you leave. Your patient advocate is here to work with management, staff and doctors to investigate and respond quickly to concerns you or your family may have. Please contact your nurse or the patient advocate at 682-3300, extension 6356 (or extension 6356 in the hospital), and we will make every effort to handle your concern as quickly as possible.

Personal Choice
The right to personal choice regarding private association and communication with other people. The patient may send and receive unopened mail unless medically contraindicated. The patient has the right to personal choice regarding participation in activities of social, religious, and community nature unless medically contraindicated. The patient may use clothing and possessions as space and unit policy permits, unless to do so would infringe upon the rights of other patients. The patient is not required to perform services for the facility that are not included for therapeutic purposes in his/her plan of care. If the patient is married, he/she shall be assured privacy for visits by his/her spouse. If both are inpatients in the facility, they are permitted to share a room unless medically contraindicated.
Freedom from Restraint
The right to considerate and respectful care, free from mental and physical abuse and free from chemical (except in emergencies) and physical restraints. Exceptions include authorizations in writing by a physician for a specified and limited period of time, and/or when necessary to protect the patient from injury to self or others.
Medical Information
The right to receive from his/her physician complete, current information concerning his/her diagnosis, treatment, and prognosis in terms the patient can be reasonably expected to understand. The patient has the right to receive information of any unanticipated outcomes of his or her treatment or care. The patient is assured confidential treatment if his/her medical records, and may approve or refuse their release to any individual outside the facility, except in the case of his/her transfer to another health care institution, or as required by law, or as third party payment contact. The patient has the right to designate a representative to make decisions in the event he/she is incapable of understanding a proposed treatment or procedure or in unable to communicate his/her wishes regarding care, and the right to access information contained in his/her clinical record within 30 days of a signed request to our medical records department.
Informed Consent & Ethical Considerations
The right to receive, from his/her physician, information necessary to give informed consent prior to the start of any procedure or treatment. The right to be included in the process for considering ethical issues that arise in the provision of his/her care. This involvement includes education as well as active participation in the decision-making process.
Right to Refuse Treatment The right to refuse treatment to the extent permitted by law and to be informed of the medical consequences of his/her action.
PATIENT PRIVACY & CONFIDENTIALITY The right to every consideration of his/her privacy in regards to his/her medical care. Case discussion, consultation, examination, and treatment are confidential and are conducted discreetly. The right to expect that all communications and records pertaining to his/her care be treated as confidential. Advanced Medical Directive & Organ and Tissue Donation The right to receive information regarding, and an opportunity to formulate, an Advanced Medical Directive, and to have hospital staff and practitioners who provide care comply with these directives. The right to receive information regarding organ and tissue donation.
NOTICE OF PRIVACY PRACTICES
THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY. We understand that medical information about you and your health is personal and sensitive. We are committed to protecting your health information and privacy. We will not disclose your information to others unless you tell us to do so, or unless the law authorizes or requires us to do so. State law requires us to get your authorization to disclose this information for payment purposes. The law protects the privacy of the health information we create and obtain in providing our care and services to you. For example, your protected health information includes your symptoms, test results, diagnoses, treatment, health information from other providers, and billing and payment information related to these services. Federal and state law allows us to use and disclose your protected health information for purposes of treatment and health care operations. This notice also applies to programs of Lake Chelan Community Hospital providing medical care, even though they may have different department names or are set up at different locations, such as Home Health Care Services, Behavioral Care Services and ambulance services and outpatient behavioral care services. Your doctor may have different policies and notices regarding the doctor’s use and disclosure of your medical information created in the doctor’s office or clinic.
EXAMPLES OF USES AND DISCLOSES OF PROTECTED HEALTH INFORMATION FOR TREATMENT, PAYMENT AND HEALTH OPERATIONS:
TREATMENT: • Information obtained by a nurse, physician, or other member of our health care team will be recorded in your medical record and used in your care and treatment. • We may provide health information to others providing you care. This will help them stay informed about your care. We may also provide health information about you to people outside our facility who may be involved in your medical care after you leave our facility, such as family members, clergy or others that provide services as part of your care, and to other health care organizations that are involved in your care via our telemedicine network.
PAYMENT:We request payment from your health insurance plan. Health plans need information from us about your medical care. Information provided to health plans may include, for example, your diagnoses, procedures performed, or recommended care. We may also tell your health plan about anticipated treatment to obtain prior approval or to determine whether your plan will cover the treatment.
HEALTH CARE OPERATIONS: We may use your health information to assess quality and improve services.
• Quality. We may use and disclose health information to review the qualifications and performance of health care providers and to train our staff. • For Appointments: We may call you by name in the waiting room when we are ready to see you. We may contact you to remind you about appointments and give you information about treatment alternatives or other health-related benefits and services.
• Fundraising: We may contact you to raise funds. • Other items: We may use and disclose your information to conduct or arrange for services, including: medical quality review by your health plan, accounting, legal, risk management, and insurance services; audit functions, including fraud and abuse detection and compliance programs.
OTHER DISCLOSURES AND USES OF PROTECTED HEALTH INFORMATION: Individuals Involved in Your Care or Payment for Your Care: Unless you object, we may disclose your health information to one of your family members, to other relatives or close personal friends, or to any other person identified by you, if we believe the information is relevant to that person’s involvement in your care or the payment of your care. In addition, we may disclose health information about you to assist in disaster relief efforts.
Directory Information may be provided to people who ask for you by name. We may use and disclose the following information in a hospital directory: your name, location, general condition and (to clergy only) religion. Clergy may be given your religious affiliation even if they do not ask for you by name. You may request to be excluded from the Directory or Clergy List upon registration or admission.
Marketing. We may provide you with general marketing information about our services or give you small promotional gifts when we see you in person without your written authorization. For example, we may send you a newsletter or a list of our health classes or we may give you a pen with our organization’s name on it. We must obtain your written authorization before we can send you marketing information about specific products or services that we provide.
WE MAY USE AND DISCLOSE YOUR PROTECTED HEALTH INFORMATION WITHOUT YOUR AUTHORIZATIONS AS FOLLOWS: Research: With medical researchers-if the research has been approved and has policies to protect the privacy of your health information. We may also share information with medical researchers preparing to conduct a research project.
Funeral Directors/Coroners consistent with applicable law to allow them to carry out their duties.
To Organ Procurement Organizations (tissue donation and transplant) or persons who obtain, store or transport organs.
To the Food and Drug Administration (FDA) relating to problems with food, supplements and products.
Workers’ Compensation: To comply with laws – if you make a workers’ compensation claim.
Public Health and Safety: as allowed or required by law:
• To prevent or reduce a serious, immediate threat to the health or safety of a person or the public • To public health or legal authorities to protect public health and safety, to prevent or control disease, injury or disability, to report births and deaths. • To report child abuse or neglect to public authorities. • To notify a person who may have been exposed to a disease or may be at risk for contracting or spreading a disease or condition.
To Correctional Institutions if you are in jail or prison, as necessary for your health and the health and safety of others.
As Required by Law: We will disclose health information about you when required to do so by federal, state or local law.
For Law Enforcement Purposes: such as when we receive a subpoena, court order, or other legal process, or you are the victim of a crime.
Health and Safety Oversight Activities: For example, we may share health information with the Department of Health, for inspections and licensure. For Disaster Relief Purposes: For example, we may share health information with disaster relief agencies to assist in notification of your condition to family or others.
For Work-Related Conditions that could affect employee health. For example, an employer may ask us to assess health risks on a job site.
To Military Authorities of US and Foreign Military Personnel: For example, the law may require us to provide information necessary to a military mission. Judicial/Administrative Proceedings: At your request or as directed by a subpoena or court order.
For Specialized Government Functions. For example, we may share information from national security purposes.
Other Uses and Disclosures of Health Information: Uses and disclosures not in this Notice will be made only as allowed or required by law or with your written authorization.
YOUR HEALTH INFORMATION RIGHTS.
The health and billing records we create and store are the property of the practice/health care facility. You have a right to:
• Receive a paper copy, read, and ask questions about this Notice; • Ask us to restrict certain uses and disclosures. However, we are not required to and do not agree to any restriction. Facility care and services are provided with that understanding; • Request in writing that you be allowed to see and receive a copy of your health information. • Have us review a denial of access to your health information-except in certain circumstances • Ask in writing to amend your health information. You may write a statement of disagreement if your request is denied. Your amendment will be stored in your medical record, and included with any release of your records. • Request an accounting of disclosures of your health information that are required by law to be maintained. You may receive this information without charge once every 12 months. We will notify you of the cost involved if you request this information more than once in 12 months. • Ask that your health information be given to you by another means or at another location. You will be asked to sign, date, and give us your request in writing. • Cancel prior authorizations to use or disclose health information by giving us a written revocation. Your revocation does not affect information that has already been released. It also does not affect any action taken before we have it. Sometimes you cannot cancel an authorization if its purpose was to obtain insurance.
CHANGES TO THIS NOTICE We are required by law to: • Keep your protected health information private • Give you this Notice • Follow the terms of the Notice that is currently in effect. We have the right to change our practices regarding the health information we maintain and to apply the change to previously received or created health information.
